803 KAR 25:110 mandates information to be reported to the executive director of OWC.  Managed care reporting documents are available for download below along with instructions for completing these reports.  The spreadsheets are posted in Microsoft Excel 4.0 format.  They may be accessed using most common spreadsheet applications including Microsoft Excel versions 4.0 through 2000 and recent versions of Lotus 1-2-3.
