Proof of Coverage

The Coverage Branch receives documented proof of workers’ compensation coverage from each carrier and self-insured group fund for employees covered by the workers’ compensation act.  Carriers and insurers submit required information using Electronic Data Interchange (EDI) through one of the state-approved data collection agents.   

  

Electronic coverage data is stored in a master database maintained by the Coverage Branch.  Certifications of coverage, monitoring of “wrap-up” construction projects, registration of employee leasing organizations and other special projects complete the responsibilities that are part of normal day-to-day operations.

Q: Where is the employee leasing application on the Web site and/or can it be e-mailed?

A: The employee leasing application is on the Web site under Forms, Form EL1 and EL2.  An application can be faxed to an employer but it must be mailed back.  This application cannot be e-mailed.

Q: Can you see if a company has coverage?

A: This information should be on the system and can be accessed if the caller has employer name, address, Federal Employer Identification Number (FEIN) and date of injury.

Q: What is a Form 4?  Where can I get a Form 4?  Can you see if a Form 4 has been filed?

A: The Coverage Branch does not handle the Form 4.  These calls will be transferred to the Enforcement Branch.

Q:  How do I get workers' compensation coverage in the state of Kentucky?

A:  Workers' compensation coverage in Kentucky is done through the voluntary market.  You should check with your insurance agent and ask him to locate a carrier that writes workers' compensation coverage in Kentucky.
