Effective June 30, 2017 the DWC Hearing Site at 410 West Chestnut Street will be permanently closed.  All Louisville dockets beginning July 1, 2017 will be temporarily relocated and conducted at the Frankfort Hearing Site, 1025 Capital Center Drive, Frankfort, Ky.

Due to ongoing development of the capacity of the Litigation Management System ("LMS") to receive and process settlement agreements in electronic format, the Department of Workers' Claims will continue, until further notice, to accept Form 110 Settlement Agreements in paper form subsequent to July 1, 2017.

A new updated 2016 Physician Fee Schedule completed the regulatory process and will become effective October 7, 2016.  In addition to physician charges, it includes changes to the anesthesia time value, G codes associated with drug screening, J codes, dental codes and charges for ground and air transportation.  This updated 2016 Physician Fee Schedule may be ordered through the Department of Workers' Claims website.
