Notes:
1. Reporting time is the time from which an employer, employer's agent or another employee first becomes aware of the fatality, catastrophe, amputation or hospitalization.
If the Frankfort office is closed and you can't talk to a person, then you must call the federal OSHA toll-free central telephone number, 800-321-6742 and report the incident. Messages or faxes will not be accepted to meet this reporting requirement.
If you do not learn about the incident at the time it occurs, you must report the incident within eight hours of the time it was reported to you, your agent or employee.
